Ordering Notes

We will need the original prescription from your doctor.

We suggest one of the following methods (you may wish to print this page for reference):

(1) Place an order, but select the option to "print an order form." Print the order form, manually fill in your credit card details, and mail the completed order form and original prescription to us (mailing address below).

(2) Download and print out our manual order form from here. (requires Acrobat Reader). Fill it in and send it with the original prescription to us.

(3) Complete your order, including credit card details, online. Then, all you need to do is send us the prescription. Please include a note quoting the order number to help us match the prescription with the order.
